Application Instructions

The CMHR Training & Education Committee will review potential candidates and select successful applicants.

New and Current Trainees should follow the application instructions below:

Step 1: Review the CMHR Terms of Reference

Step 2: Complete the CMHR Online Application

Step 3: Submit accompanying documents with your application via the qualtrics survey

  1. Curriculum Vitae 
  2. Copy of Official Transcript (NOTE: Official transcripts for institutions other than Western in hardcopy by standard)
  3. License – in the case of health professionals, a copy of the candidates’ Canadian license is required (only required for Postdoctoral candidates)
  4. Letter of support from your Primary CMHR Mentor (describe the research, research environment and why the candidate is a good fit)***
  5. Letters of Recommendation from previous mentor.***

The research proposal must be prepared in collaboration with a CMHR Mentor. The CMHR Mentor must be prepared to supervise the applicant, including providing the appropriate facilities/infrastructure, for the duration of the training

*** Mentor Letters:  Mentors are required to upload the requested letters of reference to the program site on the CMHR Submission Portal. Mentors will see a link to their own Dropbox on this site. Trainees should be identified in the document file name. Alternatively, Letters (PDF file) may be directly emailed to cmhruwo@uwo.ca (please indicate student name in subject line).

CURRENT CMHR TRAINEES

Current CMHR trainees who have transferred from an MSc to a PhD, must also use the Online Application to remain in the CMHR for the duration of their program which is available through the Application Portal. 

Additional documentation required to submit this application includes:

  1. Updated Candidate Curriculum Vitae
  2. UWO transcript – Web academic transcript or copy of Western transcript during Master's.
  3. License – in the case of health professionals, a copy of the candidates’ Canadian license is required (only required for Postdoctoral candidates)
